Every participating student and a parent or legal guardian must review the Pine First Baptist Youth Code of Conduct before completing the registration form. The online application will ask both parties to confirm that they have read and agree to follow it.

Please have the student’s contact information, emergency contacts, transportation decision, media choice, and water-activity decision ready. Do not enter medical information online.

REQUIRED PAPER FORM

Download and print the medical form.

Complete and sign one paper form for each student, then return it directly to Pine First Baptist Youth. Do not email the completed form or enter medical information online.

Two required forms

Complete the online registration and permission form. Then print, complete, sign, and return the separate Fall Retreat Medical Form. Registration is not complete until both forms are received. Never send the medical form through ordinary email or enter medical details in an online response field.

    The participating student and parent or legal guardian review the Code of Conduct PDF and confirm their agreement online.

    Online registration

    Contact details, transportation, conduct acknowledgment, general risk acknowledgment, media choice, and the separate water-activity decision and signatures.

  3. 03

    Paper medical form

    Medical conditions, allergies, medications, emergency contacts, and emergency medical authorization are completed and returned on paper.
